A 12-week guided, self-help program that teaches real-life applications and strategies for a successful transition to a more plant-based lifestyle. Utilizing the pillars of lifestyle medicine, PURE guides you to establish whole food, plant-based diet, regular physical activity, positive mindset and stress management. It emphasizes that the best way to improve your health, boost energy levels, and prevent chronic diseases is to move to a more wholefood plant-based diet that you enjoy.

How does PURE work?
Once you sign-up, THE PROGRAM WILL START THE FOLLOWING MONDAY. Check your email for a welcome note from admin@wholehabitrx.com with instructions on what to expect and how to get ready to go.
- A comprehensive assessment across all pillars of lifestyle Medicine. Based on your assessment results, you will receive:
- A customized transition plan with low-cost meals, smart grocery lists and recipe book
- A personalized lifestyle action plan for food, exercise, sleep and mindset
- Instructional health materials designed systemically in step-by-step weekly modules and easy-to-use workbooks. You will complete a new module every week over a 16-week period and track your progress along the way
- Special videos teaching weekly modules, food preparation and proper workout technique to enhance the learning experience
- Non-judgmental pantry and fridge evaluation to help you become more mindful of what are buying and eating
- Ongoing culinary support to establish new culinary habits and enhance plant-based cooking abilities
- Virtual accountability via YourCoach.Health app from any device (computer, tablet, or smart phone) to reinforce the health materials though goal-setting, problem-solving, and tailored behavioral guidance
- Motivation and encouragement via daily email
- Social support via online Plant-Based Group
- Option to purchase extra one-on-one support via YourCoach.Health app if desired/required for an additional charge.
How does YourCoach Work?
YourCoach.Health is a user-friendly, HIPAA compliant web application that will be your virtual home throughout the program. Once you sign up, you will receive an email invite to register on YourCoach.Health app and gain access to your personal program page. Within the YourCoach app, you will be able to:
- Complete lifestyle assessment form
- Set & Track Your health goals with goal reminders
- Complete weekly workbooks, logs and journals
- Access to additional tools and resources features to help you on your plant-based journey
- Unlimited in app messaging with Dr. Sneh Kumar via secure chat feature. Dr. Sneh Kumar will frequently review your workbooks including any food journals, logs, pictures or selfies you post. Providing accountability, Dr Sneh will guide you to work towards your own goals through tailored behavior modification strategies. Guaranteed response within 48 hours
- Hands-on support and assistance in various aspects of plant-based cooking and food preparation

Each live session appointment is about 60 minutes and can be scheduled for an additional charge of $197/session. During your live sessions, Dr Sneh will:
